We left Manila on a hot, humid day ... sat on the runway sweating in the cabin of the YS-11 ... then finally got airborne and headed to Baguio. After ~45 minutes of flying, the mountains came up from the plains, and we landed on a mountain top airstrip. It was *much* cooler in baguio! Ror2 expression is induced in some population of the astrocytes that receive the bFGF …more A research team led by Associate Professor Mitsuharu ENDO and Professor Yasuhiro MINAMI (both from the Department of Physiology and Cell Biology, Graduate School of Medicine, Kobe University) has pinpointed the mechanism underlying astrocyte-mediated restoration of brain tissue after an injury. This could lead to new treatments that encourage regeneration by limiting damage to neurons incurred by reduced blood supply or trauma. The findings were published on October 11 in the online version of Glia ahead of print release in January 2017. When the brain is damaged by trauma or ischemia (restriction in blood supply), immune cells such as macrophages and lymphocytes dispose of the damaged neurons with an inflammatory response. However, an excessive inflammatory response can also harm healthy neurons. Astrocytes are a type of glial cell, and the most numerous cell within the human cerebral cortex. In addition to their supportive role in providing nutrients to neurons, studies have shown that they have various other functions, including the direct or active regulation of neuronal activities. It has recently become clear that astrocytes also have an important function in the restoration of injured brain tissue. While astrocytes do not normally proliferate in healthy brains, they start to proliferate and increase their numbers around injured areas and minimize inflammation by surrounding the damaged neurons, other astrocytes, and inflammatory cells that have entered the damaged zone. Until now the mechanism that prompts astrocytes to proliferate in response to injury was unclear. The research team focused on the fact that the astrocytes which proliferate around injured areas acquire characteristics similar to neural stem cells. The receptor tyrosine kinase Ror2, a cell surface protein, is highly expressed in neural stem cells in the developing brain. Normally the Ror2 gene is "switched off" within adult brains, but these findings showed that when the brain was injured, Ror2 was expressed in a certain population of the astrocytes around the injured area. Ror2 is an important cell-surface protein that regulates the proliferation of neural stem cells, so the researchers proposed that Ror2 was regulating the proliferation of astrocytes around the injured areas. They tested this using model mice for which the Ror2 gene did not express in astrocytes. In these mice, the number of proliferating astrocytes after injury showed a remarkable decrease, and the density of astrocytes around the injury site was reduced. Using cultured astrocytes, the team analyzed the mechanism for activating the Ror2 gene, and ascertained that basic fibroblast growth factor (bFGF) can "switch on" Ror2 in some astrocytes. This research showed that in injured brains, the astrocytes that show (high) expression of Ror2 induced by bFGF signal are primarily responsible for starting proliferation. bFGF is produced by different cell types, including neurons and astrocytes in the injury zone that have escaped damage. Among the astrocytes that received these bFGF signals around the injury zone, some express Ror2 and some do not. The fact that proliferating astrocytes after brain injury are reduced during aging raises the possibility that the population of astrocytes that can express Ror2 might decrease during aging, which could cause an increase in senile dementia. Researchers are aiming to clarify the mechanism that creates these different cell populations of astrocytes. By artificially controlling the proliferation of astrocytes, in the future we can potentially minimize damage caused to neurons by brain injuries and establish a new treatment that encourages regeneration of damaged brain areas. Vitamin B12 is needed for producing and maintaining new cells, including nerve cells and red blood cells. It is also needed to help make DNA. Vitamin B12 is bound to the proteins in food. What Are Sources Of Vitamin B12? Vitamin B12 is attached to the proteins in animal foods. Breakfast cereals are fortified with vitamin B12, which is very important for people who do not consume an adequate amount of animal foods. Do I Need To Take Vitamin B12 Supplement? Studies reveal that the majority of Americans consume an adequate amount of vitamin B12. People who do not consume meat or who have a stomach or intestinal disorder that interferes with absorption may need to take a vitamin B12 supplement. Older adults may have atrophic gastritis and may need to consume additional food sources and/or supplements. There are also some medications that can influence vitamin B12 absorption, including H2 receptor antagonists used to treat peptic ulcers, metformin, and proton pump inhibitors. Your doctor can determine your vitamin B12 levels with a blood test. What Happens If I Don’t Have Enough Vitamin B12? An inadequate amount of vitamin B12 can cause pernicious anemia. What is Vitamin B12 and Why Is It So Essential? Like all vitamins, B12 is an organic compound, made from carbons (as opposed to minerals, which are inorganic), and essential for our normal metabolic function and health. Also, like most vitamins, B12 plays a wide variety of roles in our metabolism. The short list of important effects B12 has on your health includes these: Vitamin B12 is essential for the manufacture of red blood cells; a deficiency leads to a characteristic kind of anemia Vitamin B12 is needed to support the normal function of nerve cells, and to manufacture myelin, the insulating material that surrounds some of our nerve cells and speeds neural transmission Vitamin B12 is required for the replication of DNA Each of these effects is obviously quite important, but note the third one in particular. When B12 is deficient, our DNA cannot replicate normally – meaning we can’t generate new, healthy cells. As a result, vitamin B12 deficiency can mimic all of the effects of aging. Manifestations Of Vitamin B12 Deficiency Common manifestations of vitamin B12 deficiency include weakness, numbness and tingling, fatigue, dizziness, swelling and irritation of the mouth and tongue, and irritability. Anemia can develop, as noted, but a high intake of folate can compensate for a deficiency of vitamin B12 and prevent anemia. The most serious manifestation of B12 deficiency is impaired brain function, due to the effects of B12 on nerve cells. Advanced B12 deficiency causes dementia severe enough to resemble Alzheimer’s disease. Generally, though, the dementia caused by B12 deficiency is completely reversible with supplementation. While folate can prevent B12-deficient anemia, it cannot prevent the dementia – only B12 itself can do that job. Difficulties With Vitamin B12 Absorption Vitamin B12 is found in animal foods (and fortified cereals), so vegans are vulnerable to deficiency. But the most important cause of deficiency has to do with the unique way B12 is absorbed. To get into the bloodstream, B12 must be escorted by a protein called intrinsic factor, produced by cells of the stomach. Many disorders of the stomach, such as gastritis, particularly common after age 50, can interfere with the production of intrinsic factor. This condition is called pernicious anemia. Medications that affect the stomach – such as aspirin, antacids and proton-pump inhibitors – can also interfere with intrinsic factor production and result in B12 deficiency. The commonly used diabetes medication, metformin (Glucophage), can do so as well. Health-care professionals are, of course, taught to be on the lookout for B12 deficiency, particularly in patients over age 50 with suggestive signs or symptoms. But health care is at its best when both clinician and patient are on the ball! I encourage an empowered, assertive role for all patients.
